Abstract: Psoriasis is a chronic immune-mediated inflammatory skin disorder characterised by erythematous plaques covered with silvery scales and a relapsing-remitting course. The condition can adversely affect the physical, psychological, and social well-being of affected individuals. In Ayurveda, psoriasis is commonly associated with Ekakushtha, a subtype of Kshudra Kushtha, due to their similar clinical manifestations. The present case study aimed to evaluate the effect of Ayurvedic management in a patient diagnosed with psoriasis. A 29-year-old male patient presented with diffuse dry, scaly, elevated, and inflamed lesions involving the trunk, scalp, and groin region for two years, with aggravation during the previous two months. The patient also reported mild itching and absence of sweating over the affected areas. Clinical examination revealed Matsyashakalopama (scaling), Mandala (erythema), Kandu (itching), Bahalatva (epidermal thickening), Aswedana (anhidrosis) and Rukshata (dryness). Auspitz sign was positive, and the baseline Psoriasis Area and Severity Index (PASI) score was 28.1. The patient was treated with a combination of Shodhana and Shamana therapies. Shodhana included Deepana-Pachana, Snehapana with Mahatiktaka Ghrita, Virechana with Trivrit Avaleha, and Laghu Vamana. Shamana therapy consisted of Mahatiktaka Ghrita, Panchatikta Ghrita Guggulu, Aarogyavardhini Vati, Gandhaka Rasayana, Eranda Haritaki and topical application of medicated oils. After 14 weeks of treatment, significant improvement in clinical symptoms was observed, with the PASI score decreasing from 28.1 to 3.6. The findings suggest that an integrated Ayurvedic approach may be beneficial for managing psoriasis and improving quality of life.
