Abstract: Introduction: Vipadika, classified under Kshudra Kushta in Ayurveda, is a chronic dermatological condition characterised by fissuring and pain over the palms and soles. It closely correlates with Palmoplantar Psoriasis in contemporary medicine, a condition known for its chronicity, recurrence, and resistance to conventional therapy. This case is unique as it demonstrates significant clinical improvement with a conservative Ayurvedic approach using Shamana Aushadhi (palliative treatment) without adverse effects, highlighting a potential non-steroidal, holistic management strategy for chronic palmoplantar dermatosis. Main Symptoms and Clinical Findings: A 54-year-old female presented with complaints of dryness, itching, fissuring, scaling, hyperpigmentation, and bleeding cracks on both palms and soles for the past two years and six months. The condition was persistent and interfered with daily activities. Diagnosis, Therapeutic Interventions, and Outcomes: Based on Ayurvedic clinical assessment, the condition was diagnosed as Vipadika. The patient was managed with Shamana Aushadhi (palliative treatment), including both internal and external Ayurvedic formulations, along with Nidana Parivarjana (elimination of causative factors) and Pathya Ahara-Vihara (wholesome dietary and lifestyle practices recommended for the given condition). After three months of treatment, marked clinical improvement was observed, with complete resolution of cracks at some places, pain, itching, and scaling. No adverse drug reactions were reported during the treatment period. Conclusion: This case highlights that Ayurvedic management with Shamana therapy, supported by dietary and lifestyle modifications, may provide effective and safe relief in chronic Vipadika (Palmoplantar psoriasis). The findings suggest that Ayurvedic interventions can serve as a promising complementary or alternative therapeutic approach in managing chronic inflammatory palmoplantar dermatoses.
