Abstract: Urticaria is a dermatological disorder characterised by the sudden appearance of pruritic, erythematous or skin-colored wheals resulting from histamine-mediated mast cell degranulation. It is commonly triggered by allergens such as foods, drugs, insect bites, or infections and may be associated with angioedema. It is classified as acute when symptoms persist for less than six weeks and chronic when they continue beyond six weeks with recurrent episodes. Chronic urticaria may be spontaneous or inducible and can significantly impair quality of life. Conventional management includes antihistamines and short-term corticosteroids, which may have limited efficacy and adverse effects. In Ayurveda, Urticaria is correlated with Sheetapitta, Udarda, and Kotha based on the Dosha predominance. A 17-year-old male presented with intermittent itching followed by the appearance of erythematous, raised patches all over the body for the past 7 days, with a history of recurrent relapses and remissions over the preceding 3 years. He was diagnosed with Udarda considering the clinical presentation. Based on Ayurvedic principles, Samana Chikitsa (Balancing therapy) was individualised according to Dosha predominance. Clinical outcomes were assessed using the Weekly Urticaria Activity Score (UAS7) and serum Immunoglobulin E (IgE) levels. Significant improvement in urticaria symptoms and reduction in IgE were observed within 40 days without adverse effects. The findings suggest the potential efficacy of Ayurvedic formulations with Kushtaghna (Anti-dermatitis), Shothahara (Anti-inflammatory), Rasa Rakta Prasadaka (Clarifying Plasma and Blood), Virechana (Laxative) potential are effective in managing Udarda.
