Abstract: Fracture of the lower end of the radius is one of the most common fractures of the upper limb, especially in elderly individuals and post-menopausal women. Proper anatomical alignment of fractured fragments is essential for restoring wrist function and preventing long-term complications such as deformity, malunion, and restricted movement. In Ayurveda, the management of fractures (Bhagna) has been clearly explained by Acharya Sushruta, in which Sankshepa (closed reduction) followed by immobilisation is described as a primary line of treatment. A single-group clinical study was conducted on 30 patients with displaced fractures of the lower end of the radius selected from the OPD and IPD of Shalya Tantra. A closed reduction was performed, followed by plaster-of-Paris cast immobilisation. Radiological assessment was done. Parameters such as radial inclination, radial length, ulnar variance, dorsal/palmar tilt, and carpal malalignment were assessed. Statistical analysis was carried out using a paired t-test. Significant improvement was observed in most radiological parameters following closed reduction. Ulnar variance, dorsal/palmar tilt, and carpal malalignment showed highly significant improvement (p < 0.001). Radial length showed a significant correction, while radial inclination improved but was not statistically significant. Overall, results ranged from good to excellent in most patients.
