<abstract><title>Abstract</title>
Background: Dyslipidemia is a metabolic disorder characterised by abnormal serum lipid levels and is a major risk factor for cardiovascular diseases. In Ayurveda, it can be understood under the spectrum of Medoroga (disorders of fat tissue) and Santarpanajanya Vyadhi (diseases from over-nourishment) resulting from Agnimandya (impaired digestive fire) and Medovaha Srotodushti (vitiation of channels carrying fat tissue). Objective: To evaluate the effect of Virechana Karma (purgation therapy) as a sole therapeutic intervention in a patient diagnosed with dyslipidemia. Case Presentation: A 37-year-old male presented with heaviness in the chest, fatigue, weight gain, and leg pain for 8 months. Laboratory investigations revealed elevated total cholesterol, triglycerides, and LDL levels. Intervention: The patient underwent classical Virechana Karma (purgation therapy) following proper Poorvakarma (preparatory procedures) and Paschatkarma (post-purification regimen), without administering oral hypolipidemic medications. Results: Significant reduction was observed in total cholesterol (224 mg/dl to 158 mg/dl), LDL (152 mg/dl to 99 mg/dl), and triglycerides (248 mg/dl to 200 mg/dl), along with symptomatic relief and 4.5 kg weight reduction. Conclusion: Virechana Karma (purgation therapy) demonstrated encouraging clinical and biochemical improvement in dyslipidemia, suggesting its potential role in managing metabolic disorders through Shodhana therapy (purification therapy).
