Abstract: Background: Bronchial asthma is a chronic inflammatory airway disorder that can affect people of all ages and is the focus of various clinical and public health interventions. Bronchial asthma is correlated to Swasakasam in Siddha based on Yugi Vaithiya Chinthamani. Many patients are seeking treatment at Ayothidoss Pandithar Hospital, National Institute of Siddha to manage this condition. Objective: To assess the therapeutic effectiveness of Siddha formulation “Thippili Nei” in the management of “Swasakasam” (Bronchial Asthma) by using Pulmonary function test (PFT), Asthma Control Questionnaire (ACQ) and lab parameters- IgE, AEC, ESR. Materials and methods: An open clinical study of 10 patients diagnosed with Swasakasam (Bronchial Asthma), aged 18-60 years. The clinical study received approval from the Institutional Ethics Committee (IEC no: (22/12/2022; NIS/23/IEC/2022/MP/3) and was prospectively registered in the Clinical Trail Registry of India (CTRI Reg No: CTRI/2023/02/049661). The trial drug Thippili Nei 5ml was given with hot water twice a day after meals for 48 days, followed by 2 months without giving trail medicine. Results: The outcome was mainly evaluated by improvement in clinical symptoms using Asthma Control Questionnaire, PFT and observing changes in lab parameters such as ESR, IgE and AEC. The bio statistical analysis using a paired t-test before and after treatment of ACQ scores (P-value< 0.001), Pulmonary Function Test Vital Capacity, Tidal volume, Expiratory Reserve Volume, Forced Expiratory Volume FeV1, Peak Expiratory Flow, Maximal Expiratory Flow 25, Maximal Expiratory Flow 50, Maximal Expiratory Flow 75, Maximal Expiratory Flow 25-75, Elevated Airspaces (P – value
