Abstract: Introduction: Bertolotti’s syndrome, also known as Lumbosacral Transitional Vertebra (LSTV), is a congenital anomaly characterised by pseudo-articulation or fusion of the L₅ transverse process with the sacrum or ilium, leading to chronic low back pain. This condition is correlated with Katigraha, through the lens of Ayurveda, predominantly involving vitiation of Vata Dosha. Methods: A 38-year-old male patient presented with low back pain radiating to the left lower limb for 1 month. Clinical examination was done through Straight Leg Raising (SLR) test, Bragard’s Test, FABER’s test and Slump test. MRI of the Lumbar spine confirmed Castellvi type IIa Lumbosacral Transitional Vertebra (LSTV). The patient was treated with both Shamana and Shodhana chikitsa. Results: Significant symptomatic improvement was observed. Pain intensity reduced from 7/10 to 2/10 on the visual analogue scale (VAS). Stiffness resolved completely (from grade 3 to 0). Range of motion improved with no radiating pain. Conclusion: The present case suggests that Ayurvedic management, including Shodhana and Shamana therapies, can effectively reduce pain, improve mobility, and enhance quality of life.
